Here at Hometown Primary Care, we care for people where they are. We will spend time with you, making sure your needs are met. This can be in your home, over your phone/computer or in our office. We are a Hybrid DPC clinic meaning you can have a membership to our clinic and pay a set rate monthly or yearly to get the care you deserve. We also can file with some insurance companies. This practice is Nurse Practitioner owned and proud to be part of this community. I am excited to take care of you and your family. I chose this model of medicine because it gets back to the way healthcare was always meant to be!

Pricing
We are following the CMS fee schedule for our services for insured patients. Our fee-for service patients will be charged based on the service needed. The membership services are a monthly paid fee.
Insured: copay/co-insurance depending on the insurance plan. Call for further information.
Fee-for-Service (cash price) visits.
- New patient sick visit: $110
- Returning patient sick visit: $95
- DOT/CDL physicals: $100
- Sports physicals: $30
Membership costs (preventive care, sick visit, telemedicine visits, texting, calls, and emailing included)
- Adults under 55: $75 monthly or $850 annually.
- Adults over 55: $85 monthly or $960 annually.
- Couples: $130 monthly.
- Family: $160 monthly, based off two adults and two children. $20 per month per additional child.
- Single parent family: $100, based off one adult and two children. $20 per month per additional child.
IV infusions- (Buy 3 infusions and get the 4th one of equal or lesser value 50% off)
