Founder & CEO | Hometown Primary Care

Candace Herrin, APRN, FNP-C, is the founder and CEO of Hometown Primary Care, a modern primary care clinic serving patients in Choctaw, Oklahoma, Eastern Oklahoma County, and the greater Oklahoma City area. As a board-certified Family Nurse Practitioner, Candace is passionate about providing accessible, patient-centered healthcare that blends traditional primary care with innovative wellness services.

Having lived in Eastern Oklahoma County for more than 30 years, Candace opened Hometown Primary Care in 2022 with a simple mission: to bring healthcare back to a more personal, relationship-focused model where patients feel heard, valued, and truly cared for.


A Strong Clinical Background

Candace brings more than a decade of diverse clinical experience to her patients. Before becoming a Nurse Practitioner, she worked as a Registered Nurse in several high-acuity specialties, building a strong foundation in patient care and critical thinking.

Her nursing experience includes:

  • 10 years in Emergency Medicine, serving as an Emergency Department RN and Charge Nurse.
  • 2 years as an RN in a busy pulmolongy/sleep medicine specialists office.
  • 1 year in obstetrics and gynocology as a labor and delivery nurse.

After completing her Nurse Practitioner training, Candace spent two years practicing in urgent care, where she gained extensive experience diagnosing and treating a wide range of acute illnesses and injuries before opening her own clinic.


Passion for Preventive and Wellness Medicine

Candace has a special interest in preventive care, hormone optimization, and medical weight management. She has completed extensive training in hormone therapy and evidence-based weight loss treatments, helping patients improve their energy, metabolism, and overall quality of life.

She believes healthcare should focus not only on treating illness but also on helping patients feel their best and live healthier, longer lives.

Candace is committed to lifelong learning and continually pursues additional education and training to ensure her patients receive the most up-to-date, evidence-based care available.

Life Outside the Clinic

While Candace is deeply passionate about caring for her patients, her greatest joy is her family.

She and her husband (Ray) have four children, who are truly her whole world! Their oldest son graduated from the University of Central Oklahoma with a Bachelor’s degree in Musical Theatre.  He and his wife plans to relocate to California so he can pursue acting. Their second son proudly serves in the United States Army and is stationed at Fort Wainwright, Alaska. He and his wife recently welcomed Candace and Ray's first granddaughter, making them very proud grandparents.

Their oldest daughter is currently attending the University of Central Oklahoma studying Musical Theatre Education plans to one day teach musical theatre and live in New York. Their youngest daughter is a senior at Choctaw High School and plans to complete her associate degree at Rose State College before pursuing a Doctor of Veterinary Medicine degree at Oklahoma State University.

Candace’s husband has always been one of her biggest supporters and encouragers. When not supporting her he is supporting the warfighter by ensuring the structural readiness of the B-52 fleet at Tinker Air Force Base through precision depot-level sheet metal repair, helping keep the aircraft mission-ready.

When Candace does take time away from the clinic, her favorite place in the world is the beach in Florida with her family. She also enjoys listening to audiobooks, music, and taking road trips.
