What Is Primary Care at Hometown?
Primary care is your first stop for everyday health needs, from a sudden fever to a yearly checkup to a long-term condition. At Hometown, it means one provider who knows your history, has time to listen, and helps you stay well, not just get through today’s appointment.
Too many clinic visits feel rushed, with a provider who barely knows your name. Here, care keeps going after the visit ends. Members can reach their provider directly by phone, text, or email between appointments.
Hometown uses the Direct Primary Care model, which is recognized by the Direct Primary Care Coalition. Instead of billing insurance for each visit, you pay a flat monthly fee for your everyday care.
That leaves room for longer visits and real conversations.

Who Is Primary Care Right For?
Hometown is a good fit if you:
- Want one provider who knows your whole family, from kids to grandparents
- Live with a chronic condition and need steady follow-up
- Have no insurance or a high-deductible plan and want predictable costs
- Are self-employed or own a small business
- Are tired of 12-minute visits and long waits in the lobby
Employers can also offer membership to their teams. Learn about health benefits for employers.
Primary Care Pricing at Hometown
A Direct Primary Care (DPC) membership is a flat monthly fee for your everyday care. Adult memberships are $80 a month for ages 18 to 54 and $90 a month for ages 55 and older, plus a one-time $50 enrollment fee. Annual prepay options are available with savings. Not ready to join? A non-member visit is $120.
Hometown does not bill insurance, so there are no copays, no facility fees, and no surprise charges. Payment plans are also available if you would like to spread out costs. Membership does not replace insurance for hospital stays, surgeries, or specialist procedures, so many members keep a high-deductible or healthshare plan for that coverage.
